PUBLICITY RELEASE:
We hereby grant to Piano Village Foundation, FFZ Piano permission for myself or my child as a student.
• To appear on the ffzpiano.com and Pianovillage.org
• To appear in a photograph or video representing FFZPIANO and Piano Village Foundation
• To appear in a Music Teacher Association website, or other sources for your accomplishment in some events.
• These may be published in the newspaper and/or on and Felicia Feng Zhang websites or for educational/training purposes.

CONSENT TO BE PHOTOGRAPHED, VIDEOTAPED, RECORDED AND OTHER USE OF LIKENESS AND RELEASE. 
  I understand that during the course of the student's learning, that student may be photographed, filmed, videotaped, recorded, and/or that Student's picture, image, voice or likeness may otherwise be captured or copied. I hereby grant permission, without reservation, to FFZPIANO & PVF and those authorized by FFZPIANO & PVF to photograph, film, videotape, record or otherwise capture Student's picture, image, voice or likeness during the course of FFZPIANO & PVF lessons, classes, recitals, etc., and to use such material, in original or modified form, in all media now or hereafter known, with or without student's name or other information about student, in its discretion for any legitimate purpose, including promotion, public education, commercial, charitable, fundraising, and/or other activities of FFZPIANO & PVF. 
  I understand and acknowledge that neither I nor student shall be entitled to any compensation for any such use as described above. As parent/guardian of the student, I release FFZPIANO & PVF teacher or associate teachers from any and all claims that I or the student now have, or may have in the future, for any cause of action relating to the above. As parent/guardian of Student, I agree that FFZPIANO & PVF will be the sole owner of all tangible and intangible rights in the above-mentioned photographs, tapings, recordings, and/or other capturing of the student's picture, image, voice or likeness with full power of disposition. This consent shall be controlled, construed and enforced in accordance with the laws of the State of Connecticut without giving effect to conflicts of law principles. I irrevocably submit to the exclusive jurisdiction of the courts of the State of Connecticut located in Fairfield County for the purposes of any suit, action or other proceeding in connection with or relating to this consent or to the student's participation in FFZPIANO & PVF. In the event that FFZPIANO & PVF incurs any legal, investigative or other professional fees to enforce the terms of this Consent and/or prevail in any dispute with you, FFZPIANO & PVF shall be entitled to recoup such fees (including, without limitation, reasonable attorneys' fees) from you. It is the parent’s responsibility to inform the teacher if there is a change in the above general authorization. I hereby give the FFZPIANO & PVF permission for a news release and photos of myself or my child,to be used for publicity purposes in all events unless I give my writing to cancel this permission.
